Share for Care is a system where you can allow health professionals involved in your care access to an electronic summary of key parts of your general practice health records.

Your health team has many members including hospital, allied health (such as physiotherapists and occupational therapists), general practice, pharmacy, and for some, mental health staff. All have a role in working with you to keep you well.

Your main medical records are held at your general practice, but often other members of the team need that information to ensure that they make, in conjunction with you and/or your family, the best decisions for your care.

Please inform your general practice (see 'Consent Form for Release of Shared Electronic Health Record') that you want to participate in Share for Care, which gives health professionals outside your general practice permission to access a summary of your health record when they need it, helping them to work with you to make the best decisions for your care.
